Hello All,
First time here. I graduated from Top 20 University based IM program 4.5 years ago. Currently doing my 5th year of J1 waiver job in an under-served area in hospital medicine. I am expecting my green card in next 6 months and seriously considering applying for a fellowship. Anyone out here knows any one or has heard of similar career pathways for people like me successfully getting back into academics after 5 year hiatus. Any response will be highly appreciated . My areas of interest are Cards, GI, Pulm/Cc.

If you wanted to do ID, nephro, palliative care... No big deal. Happens all the time. The fact you listed three out of the four most competitive subspecialties is going to make things much, much more difficult. It's been done, but you need significant evidence of dedication to the field, whether it's research or something else.
