Just like residency...

Goto www.nrmp.org
And begin a new registration process. You will need your AAMC ID and your USMLE ID (from your USMLE registration).

Be prepared to cough up another $50.

Register for the MEDICAL SPECIALTIES MATCHING PROGRAM (you do not have to indicate which fellowship specialty).

And you should be good to go. Just remember that the site looks all too familiar so you could trick yourself into thinking that you re-registered for the residency match... but the top of the screen should read "Medical Specialties Matching Program 2012"
