does it matter where you do fellowship in terms of getting a good job (academics). Or do ple still look at residency more (like the harvards and columbias)

yes it does - do not waste your time and money in community hospital programs - do not have to chase ivy leagues either, university programs are okay. Fellowship is more personalized. Not all program directors are alike. Work / life balance programs are generally more respectful and would give you time with job interviews etc.
