has anyone heard of someone applying to a fellowship without having taken an elective in that specialty? for instance, i recently discovered i like pulm but do not have time to take an elective in the subject the application cycle this year (starts november.)

I'm sure its been done, but I'd be extremely wary for yourself to enter a fellowship you've never done a rotation in. You need to spend at least some dedicated time seeing what attendings on a pulm service do, to envision whether you could live that lifestyle. Also applies for seeing what the fellows do and what three years of your fellowship life will be like. You have to see what they do on the floors, ICU, clinic, sleep studies, etc. etc.
