I am currently at the beginning of my 3rd year. I do have a strong interest in EM, but I am curious if any of you know what kind of fellowship(s) can one do after the EM residency. What is the long term potential for specialization. Any info would be appreciated. Thanks.

Accredited fellowships are:

Peds EM
Sports medicine
Toxicology

However, there are a BUNCH more that are unaccredited (fellowships), but that does not reflect on the education - it's just that there is either no specialty board to accredit it, or an EM residency doesn't suit the prerequisites. If the fellowship PD wants you, they'll take you - you just won't be able to become board certified. You still have the knowledge, though.

One point is Peds - all peds fellowships are 3 years, as per the AAP. Some of the Peds EM programs, approached from the EM end, are only two years. Others are 3 years regardless of peds or EM background.
 
Hyperbaric (HBO) training is now fellowship required and there is a board that will certify you. Most fellowships allow you to receive a "certificate in advanced training in ...." There are several "boards" which would love your money and really, the third party payers usually don't care what "board" certifies you. In the USA, when someone says they are "board certified" they usually mean recognized by the American Board of Medical Specialties (ABMS).

Critical care (see posts in this forum) is another fellowship option.
