When do you apply for fellowships? Are there any residencies that place you on a fellowship track? I am particularly interested on perinatology. How exactly do fellowships work?

You apply for fellowships either in your last year or penultimate(the year before your last year of residency)...i have'nt heard of dual residency/fellowship programs. For more info...goto the Freida web site (i think you can get to it through the AMA home page).

Fellowships "work" as additional specialized training in your field of interest. Most require a certain amount of GME, often in 1 certain field. As noted, you generally apply for them during your final year of residency (although there are differences). Usually things like in-training exam results, letters of rec, etc. play a heavy role in obtaining a fellowship.
