few application questions

1) My school uses a composite evaluation system so I had all my recommendations sent to my prehealth advisor who then writes a composite evaluation and sends it along with the best 2 LOR's. Does he send the LOR's and matching form just to the AADSAS or to all the schools too?

2) Same as above for transcript. Tell me if this is correct: I have my school registrar send me a sealed official transcript. I then send this along with a transcipt matching form to the AADSAS? Do I need to send copies to each individual school to?

3) What exactly is the personal statement question, just "why dentistry" ?

4) What is the judicial affairs/academic dishonesty question?

5) If I'm taking the DAT in the end of June is there any reason to rush to finish my application before then?

Any help would be greatly appreciated. I read threads with similar questions and a lot have conflicting information making it more confusing.
 
1) Just to AADSAS for most schools. Some schools also require a dentist letter which you would need to send to the school directly.

2)It is best to have your school send the transcript directly to AADSAS along with the matching form you will be able to access on June 3rd.

3) Yes, the question is "Why Dentistry?" Limited to 4500 characters including spaces.

4) Not sure.

5) Yes, it seems to be the general consensus to go ahead and submit as early as possible because it takes AADSAS 4-6 weeks to verify your application before sending it out.
 
If my undergrad registrar cannot include the matching form with the transcript is it fine if I send it to myself first to attach the matching form?
