If the process of clot formation is initiated on a prosthetic valve but never completed (i.e. if fibrin strands are found on a TEE and then the patient is started on Lovenox) what happens to the fibrin? Does the body eventually break it down and get rid of it?

if the fibrin strand is completed then LMWH will not affect it by any mean.....heparin and LMWH only inhibit formation of fibrin

yes

the body will eventually break down the strands

clot formation like just about anything else in the body doesn't happen in a vacuum - go back to your coagulation cascade and you'll see that both formation and break down are actually happening at the same time all the time in the nature state and you need "something" to push the equilibrium reaction one way or the other. once you've stopped fibrin propagation, reaction is driven back the other direction.
