Floor team constantly going over cap limit

So I'll preface this by saying I haven't matched yet so I'm not asking for myself.

One of my friends is an intern at a safety-net community hospital that pretty much takes all the admissions that come in. While on floors the cap for each team is supposed to be 20 (2 interns and a pgy-2 in team). However; he was telling me that he constantly goes over cap and his team usually is covering an avg of 30 pts.

Just curious if this is normal for community programs? Is it a violation? What should one do if this is happening at their hospital? Finally, would it do more harm than good to report something like that?
 
They might be able to get by in the past few years but the culture in internal medicine is that programs are adhering to ACGME rules. Each intern 10 pts at any given time. Eventually someone will report it (a disgruntled PGY3, a prelim going to a another specialty, or an attending who has a vendetta against the PD), if the problem is not fixed, the program will likely be on probation. Ex. look at NYP Cornell in 2009. They didn't fix the problems until they were on probation. The same rules apply to university and community programs.
