In peds this is done frequently. Have you ever tried it in an adult in whom DLT placement was difficult?
 
why not just use a blocker?

For what peds VATS cases have you done this? What age range?

The handful of peds VATS cases I did were decortications. I used 2 lung ventilation with lower tidal volumes -- occasionally was asked to hold ventilation. Granted these were on kids around 4 years old. I have not done a VATS on an infant.

Difficult meaning what? Cannot get it thru the cords or cannot get it to go to left mainstem?

Anyway, no. Have not had a DLT I wasn't able to place.

I have...because the patient was tubed already....I didn't want to bother with changing out the tube...so I just shoved it in to the R main stem position.

Worked great.
