i was wondering the same thing. but i'm thinking that you won't need to fill that out until they specifically ask you to. i got a letter from LLU last week and it didn't mention filling this form out. 😕

What other schools have such verification forms? Where can I find out? I am esitmating hours right now and will have them up to par with what I am writing right now but I just want to know what kinds of questiosn I'll be asked to that I can be more aware of it as I shadow and let the dentist know too.

the verification form is just you putting where you shadowed and how many hours and it just needs to be signed off by the office.

thanks that helps! i tried looking up old threasds but coudn't find anyone else talkign about these formes but i would think more than 2 schools have them though cuz otherwise anybody could just lie about their hours if no one is giong to check on it with for example such verification forms. I'm just trying to figure out when they woudl need them to make sure I dno't over estimate. What did you guys do?
