I was wondering if the meaningful experience can be about the whole overall experience from a work/activity, or does it have to be a particular instance from that work/activity?

I think most people read it as most meaningful overall, but I'm sure the latter works too.

I read it as overall experience of a particular activity. And began it as such but narrowed it down to single particular examples of each.

A Most Meaningful designation relates to a single entry and the experience you described there. If you want to reflect on all the experiences that led you to medicine, the Personal Statement is a good place for it.
