Formulations post residency

To the attending physicians in private practice and CMH, how extensive are your formulations? Residency and fellowship has afforded me enough time to write good ones, but I fear in real world practice, efficiency is key. So I wonder how practical it is to write well thought out formulations when seeing patients every 20-30 minutes.

It's hard to get a gauge with my attendings because academia is such a different setting.

the bio/social aspect of formulation is pretty straight forward on most patients, the psychological component is usually the trickier one that I spend a few visits trying to figure out.
