The only time I use it is when I am not getting any response from the pressors any longer. I could be wrong but I have never heard a good argument for it and I am open to any opinions. Personally, I like fluids for acidosis in trauma and THAM if really needed.
Notice that the options are 1) I give it, and 2) I worry about intracellular acidosis. It doesn't say 1) I give it and 2) I don't give it. So in reading this, my answer is 2 (I worry about intracelluar acidosis), but I still give it from time to time.
