How difficult is it for a program to obtain additional funding for a training spot? What are the sources of $$$$ that a program looks at?

Very difficult. There usually is no way to do this. Occasionally via philanthropy. Someone would need to commit the entire cost of your residency, including benefits, for all of the training years.
Rarely a grant could fund a spot -- but the grant would need to be long enough to pay for all the years.
