Hello to all. I am going to graduate soon and I have big trouble deciding what speciality to go for. I really enjoy obgyn, especially obstetrics, but I am little concerned that I will get bored and stucked there. I have always planned to do research and push boundaries. Can you, more experienced colleagues, tell me how you see the field? Are there still opportunities for a big breakthrough or has the obgyn reached it's limits and now is just polishing details? Thanks a lot for answers.
 
MS4 applicant here, and will also preface you'll likely get a pretty biased opinion on an OB forum, but here goes...

I think OBGyn as a specialty is largely insulated from boredom. Obstetrics will keep you on your toes and get your adrenaline going, especially in high-risk centers. There are lots of unanswered questions with respect to physiology of pregnancy, such as confirming the pathophys of preeclampsia or gestational DM. MFM remains the preliminary route for the new field of fetal surgery, which is fast expanding with new interventions and technology. REI has vast research questions and also a lot of problems with access to care issues for poorer folks, so you could even put a public health spin on that.

Gynecologic surgery is a very challenging art to practice given the complexity of pelvic anatomy. Some places might suggest their GynOncs are the best surgeons in the hospital because of their versatility with regards to abdomen and pelvic anatomy. Ovarian cancer (especially late stage) is a huge clinical and surgical challenge, and has mega mortality. FPMRS techniques for urinary incontinence are being reevaluated and refined. And MIS surgeons in gynecology are pushing robots farther than before - even met a doc who did a tiny painting using a DaVinci, explaining how much finesse the robots have and how potentially game-changing they are.

Sorry for the bias, but I don't think OBGyn will be boring or lacks need for research at all. But it's a personal decision for everyone - if you don't think this will get you up every morning happy to go to work, it's wise to think harder about what specialty will do that for you.
