Do residencies care what you did with your gap year? If you bummed around, will that impact their decision in any way?

FWIW, I am already accepted to a U.S. medical school for 2011...I had to defer my 2010 admission for personal reasons.
 
Nope, they only care what you did in medical school.

For residency,

Step 1 score
Clinical rotation grades
AOA if your school has it
Clinical rotation evaluations

Those are prob. the biggest factors.

Other peripheral matters are leadership and research publications.

Agreed. Weak points in any one of those areas can be made up for by strong points in others. Strong in all is always better.

But, as my advisor says constantly, "Just man up and get a 250 on the Step 1 so you don't have to worry about what residency choice you want to make."

Also, building relations with residency programs of interest if you are strong in the other areas like through networking by doing elective rotations in these away hospitals if they are not affiliated with the school you are already at is an important thing.
