If I take a gap year, is my app expected to be a lot better than the traditional apps (apps for people who go to med school right after college)? What if I just continue the ecs I have been doing so far and then do a research internship after I graduate? Would that be ok?

If you don't have any research experience yet and you are going to do an internship, that's a great way to spend a gap year and it will improve your application.

By the way, you will still be considered a traditional applicant by most, if not all, medical schools, if you only take 1-2 years off.
