There are several options, but the most important element of your gap year is to make sure you do something meaningful (i.e. not McDonalds) upon which you can constructively reflect in your interviews/possibly personal statement. You could get a job in research/a lab where you would be able to explore the less clinical side of medicine, you could work in a hospital/work as an EMT, you could work in a job in another field of interest, to see if that is perhaps a better fit for you than medicine.
I work as a phlebotomist, and it gave me a lot of fodder for interviews. It also has a really high turnover rate, so it's no big shock when someone leaves.
If you are planning on becoming an EMT, I suggest you look at the market in your area.
