Getting a job or fellowship after residency- what are they looking for?

I was just wondering what makes one a competitive job or fellowship applicant after residency. Obviously letters/calls of recommendation from your residency program and interviews are important.

How much do publications matter? What other things are important? Do they still care about USMLE scores, AOA, med school class rank? How much does the "name" of your residency program matter? The name of your med school?

I realize this may vary based on what type of job you seek (academic vs private practice vs fellowship-if you call this a "job").
 
This REALLY depends on what you're applying for.

I went the private practice route, coming out of a well-known top program. Every opportunity I looked into couldn't wait to sign me up - no letters, occassionally a call to my professional references, but otherwise absolutely no questions about USMLE, mock boards scores, etc.

I suspect the other extreme would be applying to fellowship. Here, you would need to assemble your "application" - with papers, letters of recommendation, personal phone calls from powerful people in your department, etc, etc, etc.

For academic positions, a lot of times you're expected to give a lecture to the current residents as part of your interview.
