I am a 4th year medical student who is rotating through ophthalmology. Recently I came across a patient with congenital glaucoma, I was reading up on the treatment options for this disease when I came across these two procedures and was having a hard time understanding exactly what the difference between them is. Therefore, I was hoping one of you guys could hopefully clarify and elucidate on what the fundamental difference between these two procedures is. Thanks in advance.

if the cornea is clear and you have a view of the angle, then you can do goniotomy.

trabeculotomy opens the angle after cannulating schlemm's canal first, helpful when the cornea is cloudy from high eye pressure.

I'd look for videos on eyetube or youtube; seeing the surgery can really help you understand it better.
