Good resource(s) for learning how to take a good history, write a good H&P?

So, I've been out of my school's clinical curriculum for a while doing research and needless to say have seemingly forgotten lost History and Physical Exam Skills. I have my first rotation in about 2 weeks and would really like to feel comfortable with the basic H&P before I get on the wards. Can anyone recommend a good text that explains how to take a good history and physical and write one up?

I have a copy of Bates, but right now it seems quite big to get through in such a short amount of time. Are there any smaller books out there that someone would recommend. Thanks in advance,
