In PM&R: cannot go wrong with Mayo for sports.

Harvard/Spaulding, Utah, UTSW, WASHINGTON, KESSLER, TEMPLE, MCW are other strong places I found on my journey to matching.

I am sure that there are more.

MCW, MAYO, UC DAVIS, EMORY, Harvard/Spaulding, Wash U, Puerto Rico, Washington are the only places that currently have ACGME accredited sports medicine fellowships for PM&R

Stanford and RIC/Northwestern will have ACGME sports fellowships in 2011-2012

I would encourage anyone with interest in Sports Medicine as a career to use the American Medical Society for Sports Medicine as a resource.

I would add that at the fellowship level, there is also the Primary Care Sports Medicine match (can be accessed via amssm.org). In the last few years there have been a growing number of these fellowships that would consider a PM&R applicant and a few have matched PM&R residents in recent years.
