guidelines for disability requests from patients?

I occasionally get requests for patients requesting" disability" for a chronic medical condition. Are there any firm guidelines regarding what should be offered and for what duration? Some cases are obvious yes, others obvious no, and a lot somewhere in between.

If there were firm guidelines then nurses would just do it.
