Fascinating. This seeks to close the in-office exemption "loophole" in the Stark law. Exemptions would be few and far between, and this bill is pretty vague in terms of its limits (it clearly defines specific acts that would no longer be exempt like Anatomic Pathology services and Advanced Imaging, but also has a clause that states literally anything else "Any other service that the Secretary has determined is not usually provided and completed as part of the office visit to a physician’s office in which the service is determined to be necessary. ")

What are the odds this gets through this administration? I would think close to 0%.
 
This literally pops up every 4-5 years, goes nowhere because politicians have essentially zero interest in internecine warfare among specialists in healthcare and frankly the public doesnt care either.

And even if by some political fluke this makes it through, somehow Trump signs it and somehow people care enough to enforce it, there are like a million ways to get around this by forming multi-speciality groups that launder the money back to referring docs by a ton of legal methods.

THE biggest thing is how in the hell can Jackie Speier, the mistress of the congressman shot dead at Jonestown, who traveled to Guyana and saw the preparations that resulted in the single largest mass slaughter of American children (330+?) since the Trail of Tears have ANY career whatsoever in politics?

You want to get really dark, listen to the audio recording of the hundreds of children dying slowly from cyanide poison as their parents freak out and then realize that woman, that harlot of Satan himself, helped throw a dinner for the guy who did that not months before at the one of the most expensive SF hotels and honor him with a community service prize.

I cant imagine someone not in the actual Charlie Manson family, more evil yet not only walking free but actually making laws.

Kim Jong Un himself, a guy who regularly feeds people to dogs, probably would take a pass on hiring her.

The surprise network legislation is far more troubling.

Commercial insurance will likely drop payments to 50% of MC for pathology.

You won't have any ability to go out of network or even object.

It is very likely to pass unless there is a massive letter writing campaign by hospital based physicians.

Crap guys, this is is a political distraction. It will end up 125% Medicare or something and the real football is BUNDLED PAYMENTS.

Bundled payments will be the end of Pathology completely. Keep the eye on the real football.

Even if something passes it will get litigated in the courts forever.

At least Bernie Sanders will forgive all our loans in return for signing out a 5 part GI biopsy for $10.

Eventually true,but in less than 2 weeks ANTHEM will cut my payments by over half
 
Can you go out of network....
I have sent both hospital administration and my client physicians notice i may have to go non participating and why.However,going non par is fraught with political and practical perils.I also get the feeling that many of my close by fellow pathologists are just meekly accepting these cuts which makes my position even more exposed.

There is a huge cohort in our field that will, indeed, just sit and take it. Lots of folks who, by culture, are generally shy and non-confrontational and a slew of complacent non-alphas. You will ALWAYS find someone who will lo ball you in this field. This is the flip side of there being far too many pathologists.

Then i think DR REMICK and the others here who are not community pathologists and are not on the front lines business wise should be ashamed of their know it all attitude unless they are being directly affected financially.

Our group went non-par with a carrier that slashed their rates a few years back. While we received some blowback from the hospitals (especially given the "suprise billing" stories in the news), when we explained the rates offered compared to all other carriers and how that negatively affected us, they saw our side.
