H1b for residency=>J1 for fellowship, what is the disadvantage compared with J1 visa resident?

I have heard that fellowship programs are less likely to take applicants on H1b even the applicant is willing to switch to J1 for fellowship, compared with applicant with J1 for residency. What is the concern from program's perspective?

One reason is that its a lot more work and expense for the program.
 
I think it is the ECFMG that will do paper work for J1 sponsorship, and the fellowship program does not need to pay.
The only concern that I have heard is that resident on H1 visa may dump the fellowship program if they can find a green card sponsoring job before graduating from residency.
Is this correct?
