Step 3 is required for h1b - Not for j1.

However completion of step3 does not make it "very likely" you will get a spot - competition for visa sponsored spots is fierce, and imgs are at a disadvantage for any spot compared to us grads.

If you do apply, apply broadly to many programs to increase your chance of getting an interview.

Not very many residency programs offer H1b in general - it's much more expensive for the program than a J1, as well as much more of a hassle. With a J, the applicant (along with the ECFMG) does almost all the work themselves. With an H, the program has to file the paperwork, pay the fees, etc.

IMGs in general have a harder time matching residency than AMGs. Limiting oneself to *only* programs that sponser an H1b doesn't making anything "very likely".

Would you still say it's more likely for a Swede with good step scores ( >249 step 1) who applies for 75 % H1B-residencies (rest 25% J1) with the prerequisite of step 3 done to ultimately end in a J1? How should the ideal distribution of H1B/J1 residencies (in application) look like?
 
This is hard to answer. You might find that the "best" programs that you get interviews from are those that offer J visas. If you decide to rank all H visa offers first, then your chances of getting an H are increased.
