Help: Filling out the Application Form

Hello,
I'm a medical graduate from Pakistan. My exam was held on the 16th of Nov 2011 and the result got announced on some date in January 2012. I haven't been given the Degree/diploma/certificate for my MBBS, yet and while filling out the forms for step 1 needed help for the following spaces.

1) Date you completed requirements for the final medical diploma
2) Number of years attended
3) Graduation date
4) Date medical diploma issued

I'll be grateful if you guys can help me out on this.
