IMHO, 3>2>1. I think convincing someone to do something is much harder than self-discipline (what weight loss entails), and a very admirable skill to have as a physician!
Don't worry about your topics seeming "not significant." The prompt asked for greatest personal achievement. It's not a contest to see who had the toughest life so far. Just explain why it was the greatest achievement to you.
