@Goro @Faha Do I have a shot at T20 schools/should I bother applying? I'm ORM and MCAT is a little low for these schools. I also don't know if my gap years in consulting will be viewed favorably considering my firm is popular with big pharma

Suggest deleting the SUNYs, adding the other NJ schools, JHU, Albany (maybe), U MD (maybe), Vandy, U MI, UVM, Gtown (maybe)
 
If you are thinking MD/MBA you should add Yale, Stanford, and Uchicago to that list. (If being strict then take off Hopkins bc of MBA interests).
 
To OP, IMHO, you should shadow primary care a bit. Take this with a grain of salt. I am but a lowly pre-med.

Granted, I am a premed as well, but our adcoms have reiterated many times that longitudinal primary care shadowing is strongly preferred. I think it was gyngyn (she's a practicing OB/GYN and admissions committee member for those who don't know her, OP) who also grows concerned when an applicant only shadows in surgery as OP has, as surgeons in the OR do not represent what docs, even surgeons, do the vast majority of the time.

I think its worth a shot. I had zero research on my app and it worked out. Probably depends how you spin your app. Again these are more worth considering since you are thinking MD/MBA. Also, Yale and Harvard have a combined agreement with Stanford GSB where we can do the MD/MBA in 5 years rather than the traditional 6 if you are set on doing the MBA there.

There are always more things you can do to beef up your application, but stay encouraged through this process! I was a nontrad applicant and was able to get accepted in my first cycle with mediocre grades and mcat.
