I was just wondering how competitve these are-- especially for an american FMG coming from a good medicine program.

Heme/Onc isn't terribly competitive right now. Of course, like everything else, the top fellowships are extremely hard to get (Sloan, MD Anderson, Dana Farber, etc.) The really competitive programs require a tremendous amount of research experience before fellowship as well as committment to continue scholarly activity while in the fellowship. There are many community based or even major university based programs which will train you to become a well rounded oncologist and enter private practice if research isn't your thing. These aren't all that hard to get and coming from a good program (like you said you are) will definitely help but isn't completely necessary.

I would think it depends entirely on where you train for internal medicine, and also to a good degree who is writing letters for you and making calls on your behalf (ie the heme onc people at your residency program).
My 2 cents.
