HIPAA compliance in private practice

Hello all. I have been following a couple of private practice adventurers on the forum and I wad curious how is everyone taking care of the HIPAA compliance policies, such as having the written down policies (not just the practice policy handed to the patient), security risk assessment and such to make sure everything about the practice is in compliance with HIPAA. Thx
 
We are a large (250 employee) agency so have a variety of people involved. We have the compliance officer who is also our COO. All new employees take a live, in-person HIPAA class as part of orientation before allowed to do ANYTHING. This includes all students and observers. Every year all employees must complete a 1 hr HIPAA video and pass the 15 question test. If you miss your date of renewal you are not allowed to work until you have attended the live class again, then do the video and test. Policies are written and some are part of all new patient paperwork.
We also have 2 full time IT workers and one EMR worker who address any tech issues. All sections within our agencies have monthly staff ,eetings and discuss any HIPAA issues, review needed policies prn.
