I would choose where ever you feel you'd be most likely to succeed. Whether that means being close to family, continuing research at your home instiutute, going to a program with your desired fellowship, etc. Also, at the top tier (MGH, Hopkins, UCSF) you can go to the fellowship of your choosing.
