In addition to what people post here you may try a search. 1 or 2 years ago there were a bunch of great posts comparing the two. Many people had great comments about bayview.

I was very impressed during my Bayview interview. The program directors are amazing, everyone seemed to love Hopkins although the residents seemed a little tired. I also frequently heard that the downtown program was more "malignant" than Bayview, the interns have to wear shorter white coats than the residents. The medical students I spoke to had a more favorable impression of Bayview compared to the Osler program. Fellowship matching from Bayview is terrific, so I really don't see any drawbacks compared to the other Hopkins.

It's NOT Hopkins. Seriously, this is the drawback. For your entire career, when asked you'll continually have to say, "No no, the other Hopkins...Bayview."

I'm just sayin'...

While that's true, the medicine faculty are pretty well split between the two campuses. You can say you worked with Dr. X rather than stressing where in Baltimore the internship took place, if that sort of thing matters (and it might for fellowship or whatever).

Main real difference is the patient population. East Baltimore (the "original" hopkins) gets more inner city and african american patients, has a busier ER and more of the problems that come with being smack dab in the middle of America's (former!) homicide capital, but you also get to bump heads with some of the prestigious specialty services more often (whether thats an advantage or a disadvantage you can decide for yourself). Bayview has more elderly patients, there's a big East European (russian, ukrainian, polish) population, has more of a community hospital rather than inner city hospital feel to it, and the campus is more sprawling. Both great hospitals to work at, IMHO, in either case its a good mix of underserved and people accessing Hopkins care cause they can. Based on my observation, the maligancy of the East Baltimore (Osler) program is overrated, its frontloaded and hard, but that's residency, and they don't seem to be flaming out. That being said, Bayview is where med students go for the "easier" medicine clerkship, can't say that translates too much to residency programs but its slightly more relaxed (less from faculty pressure than from the overall feel of the hospital, I think). People match into great fellowships from both, though Osler is more "prestigious." I'm looking at medicine/fellowship and would love to match at either.
