How can I ask.

Hi my name is _ and I am interested in applying to medical school. One of the requirements to apply to osteopathic school is that I shadow an osteopathic physician. I was wondering if it would be possible for me to shadow you for me to better understand the role as a physician from a physician's point and get a better understanding of osteopathic principles... Something like this should be fine. Most DOs know that applying to osteopathic medical school requires a recommendation letter since they went through the process themselves. This is just a quick example of over but feel free to use as much as you like or change whatever

Just ask. Be concise and courteous about it. They remember being where you are now.

A lot of them don't seem to remember though... I contacted at least 20 DOs before one let me shadow her.

OP- I recommend you target DO's who have private practices. A lot of hospitals seem to have rules against shadowers

Whatever path you go, indeed be professional. Cold calling is going to probably have the lowest success rate. Instead, work your contacts. Ask friends/family/co-workers if they know any DOs (whether personally or professionally). See if they can break the ice for you (i.e. next time they see them, they mention you would like to shadow, can you contact them, etc).

A doctor who is open to shadowing (but may be hard to get a hold of via cold calling) will probably be more receptive if someone they already know (patient, friend, whatever) vouches for you first.

Social connections work wonders.
