You don't have to rank all the places you interview at and neither do they. If there's someplace you absolutely don't want to go, you don't rank it. Likewise if a program absolutely doesn't want that applicant in their program, they don't have to rank them.

Absolutely not. Programs will rank as many applicants as they think they need to to safely fill - although in the past, some have misjudged this and ended up not filling.

But programs are just like applicants - they won't rank someone they don't want either.
