How does the match work in this case???

Lets say (hypothetically of course) that I'm applying for neurology and pm&r. Also lets assume that I am an osteopathic student and would like to reserve full practice rights in all 50 states. In order to reserve these rights I must do a traditional osteo rotating internship. Now, all pm&r programs whether allopathic or osteopathic accept the traditional osteopathic rotating year. However, allopathic neurology programs will NOT accept the traditional osteopathic rotating year.

So lets say for intern year I rank traditional osteopathic rotating years(which pmr will accept) and dually accredited allo/osteo rotating intern years(which allopathic neuro programs will accept). I also than rank neurology and PMR programs. What happens if I match into one of the traditional osteo rotating years thats not dually accreditited but than also match into an allopathic neurology program? What happens at that point???

you would be in violation as you would not be able to start your allopathic neuro program which you agreed you would be by using the match.

Therefore you cannot rank any pgy1 year that would not qualify you for a given prgram.

In other words you can avoid this situation by properly setting up your rank list.

If program A (Advanced pgy2-4) requires a certain things for pgy1 then your supplemental list (ie your prelim list of programs you link with the advanced program) must only include programs that meet the proper requirement.
If advanced prgram B allows for dfferent requirements you link a different set of prelims with that one
 
Two points in addition to minstrals comments:

1. Unless things have changed in the last two years since I applied, I did not find any free standing, dually accredited internships. The dually accredited internships that I found were all part of categorical FP or IM programs. Matching into any of these programs would get you pulled from matching in any other PM&R or Neuro spots. I have since found one program that gave an intern such a free standing dually accredited internship, but he had been a student at that institution and worked this internship out with the DME prior to the match.

2. Have you found any AOA only internships that you can rank through the ACGME NRMP match? That would be necessary for you to do what you are describing. It sounds like you're planning on ranking Neuro and PM&R programs through the ACGME NRMP match. In that case all the internships would also need to be ones you could rank through that match. If you ranked an AOA program through the AOA match and got it, you would be inelligible for any Neuro programs through the ACGME match. ABPN Guidelines require the ACGME accredited internship for their Neuro programs.

I don't see how you can make your plan work for Neuro. There's always resolution 42, a dually accredited Neuro program or forgetting about "those 5 states".

i guess i didnt realize he was talkin about pgy1 programs not accessible through ERAS match.
Sounds like the OP needs to research things more thoroughly so he knows how things work and what he needs to do.
 
Correct me if I am wrong but isnt Nassau University Medical Center's rotating internship dually accredited? What about Griffen hospital in Connecticut? I have heard that such dually accredited programs exist. Does anybody know the definitive answer to this? The answer to this question will really help me solve the issue at hand.
