How hard is it to set up a psychiatry private practice?

I'm interested in psychiatry, particularly child and adolescent psych, and am wondering how hard it is to set up a psychiatry private practice?

Where do referrals come from?
What types of work schedules could one sustain a solid practice with?
What will be your biggest bread and butter money maker in the practice?
Would one have a lot of free time to still enjoy life?

One thing that I am aware of, though, is that at least anecdotally the economy appears to have negatively impacted the number of higher end full-fee private practice cash clients [$200+/hour] for some of my mental health professional friends. Still everyone seems to be doing fine -- or at least well enough.
