How many times a year is the USMLE Step 1 exam offered?

Incorrect. You get to choose three-month period with usmle, then book a day with prometric. You can extend your three-month eligibility period until it has expired for a fee. You can re-book the exam with prometric, as long as you new chosen date falls within your eligibility period and it is 5 days before your proposed test date.

I plan to take the exam mid-June and completely forgot that it's recommended that you apply 6 months ahead. I submitted the initial application today. Will I still have the luxury of choosing the specific date(s) that I want to take the test? Thanks in advance.
 
You should be fine. I'm not sure when I registered, but it was well after the "recommended time" and I still had a choice of several dates in my desired timeframe. I think I ended up registering around mid March (!) for my June test date, but I definitely don't recommend waiting that long. 😳 Unless you have only one particular day that will work, you should be fine. Don't worry, just get the scheduling done as soon as you can.
