How strict are prerequisite course requirements?

I'm guessing this varies by school, but I'm wondering what the general consensus is.

For example, I was planning on applying to Arizona. They have a requirement of 1 physiology course and 2 additional biology courses. I have taken multiple biology courses (molecular biology, organismal/evolutionary biology, genetics, mechanobiology) but I have not taken a physiology course. Would they ignore an otherwise acadamically sound (3.9 GPA, high 30s MCAT) because I have not taken a physiology course? Or would one of the others (perhaps organismal bio) satisfy this?

Some other schools (Stanford for example) explicitly state that their requirements are not strict, but are just recommendations.

They're most likely not going to waive some requirement that a bunch of doctors with administration positions sat together and decided on because some one is smart on a standardized test (MCAT) and knows how to comprehend material of that student's choosing (GPA).

If it's required and you don't have it completed by matriculation, it is possible to have your acceptance withdrawn
 
Why wouldn't you take physiology anyway? You're going to need that knowledge in medical school. JUST TAKE IT!!!!
