How stupid would this be?

I really want to do residency at Yale, but there's a good chance I'll want to do a fellowship in pain, and Yale doesn't offer that.

I know there's no guarantee I'll match at Yale in the first place, but I'll be very unhappy if I don't for one reason or another.

Let's say I do match there though...would that make it much more difficult for me to get a pain fellowship somewhere else? Should I just try to get into a program that has its own fellowship, even though I'll be unhappy for 4 years??

go to Yale. It's a 'big name'. Regardless of whether it has a pain fellowship (which it didnt transiently), it'll still look good when it comes time for applying 'far and wide'.
 
why do you REALLY want to go to a specific place (like Yale)? There are lots of big name institutions in that area and are located better (Boston, NYC). If you look at it though, places tend to take their good residents from inhouse for fellowships (check out columbia and see how they take virtually all of their people from there). Not the same for Yale - Just check out their website and see how many stay at Yale (and we don't know whether this is by choice or what).
 
Go to Yale. Ask for an outside rotation at your favorite pain program.
 
Its important to go where you will be happy, its 3-4 years of your life and you want to be happy both at work and outside of work. Yale residents have always done well in applying for fellowships no matter what the specialty.

Simple. If you don't mind moving around for the pain fellowship year then Yale is fine. If however you would like to remain in the same location and not move around then Yale is a poor choice.
