how to apply in mult cities

hi everyone,
just wondering what ppl would suggest for someone who has to apply to multiple cities initially for residency (and narrow them down later based on where a spouse ends up for grad schl). i thought it would be 2 cities (chicago, houston), so i applied to 14 total, but we just added 2 extra cities (ny, boston).

my step 1 scores arent that great. how many programs should i add for each of the other 2 cities as far as applying. how many interviews should i do total (ball park)...what would be normal?
