how to become medicaid provider

i am trying to obtain medicaid provider id number in PA, i have been searching the web, but cannot seem to find any information about it. does anybody have information on about the application process???

Usually there is a paper application where you have to fill out information related to your education background, SS ID, address, work profile, type of dentistry you do etc.

Better to look for a sign to put in your office that says 'We do not accept Medicaid/Medicare'
