How to get a better upward trend

So, my first semester in college, I managed to get a 3.75 with out any problems. Following that semester I transferred schools, to a bigger university (IU) and I have dropped to a 2.611 cGPA which is not the best by any means, and it is all my fault for the lack of time management. The question I want answered is, how much of an upward trend would be significant enough to better my chances? Obviously straight A's from here on out would be great but as the semesters go on it doesn't get any easier. Also, any tips on bettering study habits, staying motivated, and focusing more?
 
You said it...straight A's from here on out. If you continue the downward trend you started kiss med school goodbye. That should be your motivation. Don't overload yourself with hard courses and a ton of credits.
