How to rank when Applying to 3 specialities

I'm applying to three different specialities, but I'm not sure how the ranking works. Do you make a primary list and two supplemental lists?

How about if your applying to an advanced and a categorical specialty? Which list becomes primary? Has anyone done this, and could they give their advice?
 
Your advanced and categorical ranks all go on your main list. Presumably (based on your user name) you're most interested in rads. You would then rank all of your rads programs, in whatever order you'd prefer. After that, you'd rank all of your categorical programs (perhaps categorical IM). After that, you can list all prelim programs (if you'd want a prelim only match). You'd then create prelim lists for prelim programs that you would link to your advanced matches.
