How to tackle the goals in medicine question?

Just curious how to approach secondary and interview questions where they ask what are your future goals in medicine? I dont know for certain what field Im interested in but have an idea I would like to be in academic medicine. When they ask what your goals they seem like they want more than just surface stuff. How have you approached this question in the past? Thanks!

they don't need specifics if you don't know specifics....i've been asked this at many interviews, and i've said just that : that i'm going in to medical school with an open mind and don't yet know which field i want to end up in, but i have done a lot of research and have enjoyed it greatly, and i have somewhat of an idea that i'd like to continue research in medical school and do academic medicine in the future.

all of my interviewers were fine with my answer, and none of them gave me a hard time about it.
