How to use patient name in application?

One of my essays reflects on my interaction with a patient. I obviously changed his name and put it in but I'm scared that people might think I just disclosed personal information?

The "John (pseudonym)" approach is clunky.

Is it okay to write " During my conversation with Mr. X..."

or "X"

or "X-"

Or should I use initials like A.B?

It's honestly looking kinda off so I would be great if you could reassure me that it's normal, or suggest what is the least "off" way to write this

Put John, JX, or Mr X in quotes and it will be clear it's a pseudonym. My preference is also "Mr X."

I wrote mine like ‘Sue’ because Sue stands for pseudonym and I thought I was being clever lol

As long as you clearly indicate it is a pseudonym to start (ie, parentheses, quotation marks etc.) then you should be GTG. Only use the. Patient’s name though if you refer to them more than once.
