How to write an addendum?

Can any1 suggest or know how to write an ddendum? I ommited something on my AADSAS app that I would like the schools to know. How should I structure the letter? Also would I mail it to the same address that I would mail the checks too (the one listed for the schools on aadsas) or should I contact each schools individually and ask for the address on where to mail an addendum? Thanks.

Mail it to the dental school admissions office. This is not necessarily the same address as where your money goes.

I am writing to supplement my information submitted with my AADSAS application. With regards to the Financial Background section question regarding wallet size. It turns out that I had to upgrade from a SUV to the original Hummer just to have enough space for my money and all it's friends. It can all be yours if you let me in.

The one above works great- I wrote one that was a little less formal. It said something like:

Harvard Dental School
123 Exeter Ave
Boston, MA 34343

To Whom it May Concern:

Since submitting my AADSAS application I have registered for a graduate level class in yada yada and will be completing this class during the Fall of 2006. I have also accepted a position at the blank blank lab under Dr. whatshisname. Thank you for noting these changes.

LMAOOO. You guys are the best!
So I should basically call each school and ask them for the mailling address where I can send them an addendum?
 
I would look at the AADSAS list of dental schools. If it looks like a general campus address not at the dental school, then check their web page for mailing information. If neither of those work, then I would call the school. You could also send the information in with the secondary.
 
thanks guys. I think Im going to include my AADSAS but not social just in case it falls into the wrong hands.

Im thinking of putting the letter together (in the same envelope) as the application fee check. That way I will know if they recieved it or not if they cashed the check etc... Do you think its a good idea?
 
An excellent choice. However, some schools wait months before cashing the checks. If you are OCD, this could be a problem.
