How will we show to the dental schools that we shadowed a doctor or volunteered?
Am I supposed to ask the doctor to write me a note or what?

I created a log and had the doctor sign it at the end of each session filling in the hours. I never needed it though, mostly it is on the honor system.
 
Most schools require a letter of rec from a dentist that you have spent some time with. In his/her letter, he/she should be able to illustrate the amount of time you have spent shadowing or assisting.
 
Have your dentist write you a letter of recommendation. If you shadowed for 40 hours then you put it in AADSAS when you fill out your application.

When your dentist writes you a letter of recommendation fill out a litle BIO sheet for him about yourself. Grades, extracurricular activities, hobbies, the hours you shadowed, etc. It helps him fill out your letter. Have him also mention the amount of hours you shadowed.

When admissions looks at your app they will see that you have shadowed 40 hours and they will see proof of it in your letter of rec. This makes you CREDIBLE.

You should do this for most of your letters of rec. If you tutored in math lab have your math boss write you a letter of rec. etc. (no you do not have to get a letter or rec for EVERY thing in AADSAS, just pick the best couple or so.)

The person Ive been dental assiting for 3 years for is my uncle, and we all know how its a major faux pa to have your uncle write you a letter of recommendation. I have been shadowing another dentist (an OMFS) but ive only done that for about 12 hours or so. Any recommendations?
 
Oh yeah! That letter will certainly get you a Harvard interview guaranteed.
 
make sure you check the websites of the schools you are interested in because i know that Ohio State has a specific form to fill out

What if I am shadowing 5 doctors, should I ask all of them to write me a recommendation letter? How many do I need from each?
 
5 is overkill. Shadow one or two and make friends with them. You'll get better letters that way.

I think shadowing so many people may make you a STALKER.

I mostly shadowed my cousin, who also wrote my LOR. He is a pacific graduate which didnt hurt but I did receive other interview offers so I dont think it matters a whole lot. They just want to see you have dental experience.

I volunteered in both a dental clinic and in the OR at my local hospital. I also shadowed around my dentist.
Some of it has to be dental related, but not all of it. They want to make sure you know what the dental field is all about before they commit to you.
 
If your GPA is only 3.8 and your DAT is only a 24 you are probably going to have to supplement your app with 8 dentists and 1000 hours of shadowing.
