She replied saying she would gladly write me one. My only concern is I don't think she knows me that well to write me a great one(she knows who I am since I visited her office hours and asked questions after class, but she doesn't know me too well). In my email I asked if it was possible to schedule an appointment to discuss the LOR further but she didn't answer that question. She just said "Yes, I will gladly write you a LOR". It has been a month since she replied saying "yes". Should I email her asking to schedule an appointment or just do nothing and see what happens?

Professors generally ask for your CV and PS as well, why don't you send her those?

I would send her a resume (or CV) and PS in a follow-up email AND ask if it would be possible to stop by her office sometime to discuss the LOR further, you can also use that as an opportunity to give her more specific instructions if she needs them regarding submission to your school's pre-health committee or uploading to AMCAS directly using your AMCAS ID
